Please call St. Anthony of Padua Parish Outreach at 631-261-1695 to schedule your appointment and to get information on how to complete required forms.
Bring the following to your appointment:
1. Photo ID.
2. Current insurance card. Those without insurance can still receive the vaccination.
3. Completed forms (a Proof of Eligibility Form and a Screening and Consent Form).
They can be found at www.visitingnurseservice.org/covid-19/. Recipients will be issued a vaccination card, and your Moderna second dose will be scheduled for June 11 at the Parish Outreach office.
